Findings:
A double crown is present on the UL5, this is either a gemination or a fusion between the tooth and a supernumerary tooth. There is a small crown attached to the mesiobuccal surface of the normally sized crown, with a deep groove separating them. There is a single root with a single root canal but a pulp branch splits off towards the small crown. The root apex sits close to the buccal cortical plate. There is a small area of periapical inflammatory bone loss, with perforation of the cortical plate.
Buccal exostoses are present on the buccal cortex overlying the UL4 and another linear exostosis overlying the buccal side of UL56 and UL6. Small exostoses are also present on the palatal surface of the alveolus in the left molar region. Multiple exostoses are occasionally seen as a variant of normal, which is the likely situation here. They can be familial or reactive.
