Findings:
- There is generalized mild horizontal periodontal bone loss.
- The LR4 has an atypical root morphology with a long pulp chamber and three roots – mesial, distal and lingual (Image 2).
- The mesial root has a single canal that splits at the apical root third into a delta shape. There is a periapical low-density lesion, consistent with apical periodontitis (Image 3).
- The distal root has a canal that splits to two canals (DB and DL) at the cervical root level. There is a periapical low-density lesion, consistent with apical periodontitis (Images 4 and 5).
- The lingual root has a single canal that splits into a delta shaped canal at the apical third (Image 6).
- There is a low-density area at the furcation region, associated with lateral accessory canals consistent with inter-radicular periodontitis.
